Conjugated bilirubin is also commonly known as direct bilirubin, although the two terms are not strictly equivalent. Bilirubin exists in two forms; unconjugated and conjugated. Unconjugated bilirubin is insoluble in water. This means it can only travel in the bloodstream if bound to albumin and it cannot be directly excreted from the body. In contrast, conjugated bilirubin is water soluble. Bilirubin must be conjugated to a water-soluble substance. This increased its water solubility, decreases its lipid solubility and eases its excretion. Conjugation is accomplished by attaching two molecules of glucuronic acid to it in a two step process.
